A printable order form is available through the Ordering Connection page link - it works this way: As you view items you wish to order, click the "add to cart" button and the order form will fill out automatically as items are added to the cart. The "view cart" button will show what items are in the cart. At the bottom of the "view cart" page, the "place order" button leads to a checkout page. There you'll have 2 choices: 1) ) Print the form out by clicking the "Fax/Mail Order Form" button and send with a check, money order or credit card information (VISA, Mastercard or Discover Card) to the address indicated at the bottom of the form, or 2) Order directly online with a VISA, Mastercard or Discover Card ("Proceed to checkout" button).
